Clarity Mental Health is a mental health clinic located in Clarksville, TN.  We offer medication management for the treatment of all mental health conditions, such as ADHD, depression, anxiety, bipolar, schizophrenia, and PTSD, among others.  At this time, psychotherapy is available in special circumstances only, but we continue to work closely with therapists in the area to ensure our clients have access to all of their therapy needs.  At Clarity Mental Health we believe the patient should be an active participant in his/her treatment, and we will make every effort to find a treatment option that fits your lifestyle best.

Practice Areas and other important information

Clarity Mental Health specializes in a variety of mental health diagnoses.  We treat patients ranging from age 6 and up, providing you and/or your family with medication management throughout your treatment.

Psychiatric Evaluation

All patients receive an initial psychiatric evaluation ranging from 45-60 minutes.  We take the time to listen to your concerns, and work with you to develop a treatment plan that fits your lifestyle and your personal goals. Please see our New Patient section to access new patient forms prior to your initial appointment.

Psychopharmacology

If medication is indicated for your particular diagnosis, we will work with you to determine the best fit for your lifestyle and your budget. Once your initial evaluation is complete, follow-up medication management appointments typically last between 20-30 minutes, and are scheduled monthly until you are stabilized.

*Note: In accordance with policies set forth by the Drug Enforcement Agency (DEA), patients who are prescribed controlled substances are required to receive periodic urine drug screens to ensure medication compliance. Medication non-compliance or refusal to provide a urine sample may result in immediate discharge from the clinic. Review our controlled substance policy here.

At this time, limited psychotherapy options are available for special circumstances.  We hope to add an in house therapist(s) in the future, but in the meantime we are working closely with therapists in our community to ensure you find the perfect fit for all of your psychotherapy needs.

NEXUS Letters

We are now offering NEXUS letters for veterans seeking mental health-related disability benefits. Our office works closely with you to create a detailed, personalized letter that clearly establishes the connection between your military service and your mental health conditions. Whether you're dealing with PTSD, anxiety, depression, or other service-related mental health challenges, we ensure your case is supported with the clinical evidence needed to strengthen your claim. Trust us to help you get the recognition and benefits you deserve

* Note: Please contact the office for additional information. We can provide you with a list of what is needed to get started with your NEXUS letter. The cost associated with these letters varies based on the number of mental health conditions and the extent of the records we will have to review. Please allow 7-10 business days for the completion of your letter.

Disability Paperwork
Benzodiazepine Policy

Upon completion of a thorough psychiatric evaluation, if the use of benzodiazepines is indicated for the treatment of your diagnosis, we ask that you review and sign our Benzodiazepine Treatment Agreement.  All patients with an active benzodiazepine prescription are expected to maintain regular appointment attendance, and may be subject to urine drug screens.  

We understand your disability can be life changing, and respect the need for you to have proper documentation to support your disability claims.  Due to the high volume of patients who require disability paperwork to be completed, please allow 5-7 business days for paperwork to be completed and mailed. Please review our office policy for disability form processing.

Cancellation & No-Show Policy

We require a 24-hour notice for the cancellation of all scheduled appointments. Appointments can be cancelled by calling the office front desk at 931-245-1500, or by sending an email to info@getclaritymh.com.  If you are an established patient, you may also cancel your appointment by logging in to our patient portal and sending an email to your provider.

Screeners, Forms, & Policies
New Patients

New patients are invited to set-up an account on our practice portal. Once your appointment has been scheduled, you will receive an email with information on how to access your patient portal.  Please take the time to set up your account and review our practice policies, located on the left side of your portal under "Documents."  

 

If you are unable to access the portal, please click here to complete the new patient forms required prior to your initial appointment.  Forms are available in .pdf format and may be downloaded, filled out, and brought with you to your first appointment.  

 

Completion of the new patient forms prior to your scheduled appointment helps us ensure a timely schedule throughout the day.  If you have questions while completing the forms, please contact our office at 931-245-1500.

Telehealth Services

IWe offer Telehealth services via our EHR, PracticeQ, for follow up appointments only, all new patient initial appointments must be done in the office.

 

Our EHR is a HIPPA protected Telehealth platform that enables you to meet with your provider from the comfort of your own home. 

​

Prior to your Telehealth appointment, you will receive a link to our virtual waiting room in your patient portal, and also to the email/phone number you provided during your initial evaluation. Please be sure to remain in the virtual waiting room until your provider arrives; we appreciate your patience.  For connection issues, contact the office at 931-245-1500.

Note:  Patients who are prescribed controlled substances will still be required to comply with our controlled substance policy, to include the collection of random urine drug screens.  You may be required to periodically visit the office or nearby lab in order to provide us with your urine sample.
